Common triggers include\u00a0<strong>alcohol<\/strong>, certain\u00a0<strong>medications<\/strong>, and\u00a0<strong>infections<\/strong>. Diagnosis is through a\u00a0<strong>physical exam<\/strong>\u00a0and sometimes a\u00a0<strong>biopsy<\/strong>.
